The study of cognitive disorders (CD) began with more severe forms - dementia, which, as a rule, is the result of a long-lasting pathological process. It is preceded by non-dementia cognitive impairment (subjective, mild and moderate), the early detection of which expands the potential for therapeutic intervention and prevention of dementia. The article provides an overview of modern publications on the problem of non-dementia CR. Historical issues in the study of these disorders are considered, modern principles of their classification and diagnostic criteria are given, and the main aspects of patient management are described. Particular attention is paid to the issues of diagnosis and treatment of the clinically heterogeneous syndrome of mild cognitive impairment (MCI). The results of a number of Russian and international studies of MCI syndrome, including our own work, are described. Much attention is paid to the issue of differential diagnosis of physiological and pathological changes in cognitive functions. The results of our own study of the dynamics of cognitive functions in people aged 50–85 years in the absence of neurodegenerative, vascular and other diseases significant for cognitive activity are presented.
