The article analyzes data from modern literature on the optimal timing and methods of delivery of twins. Recently, multiple pregnancies are becoming much more common, which is mainly due to the active introduction of assisted reproductive technologies. The risks of perinatal and maternal complications increase with multiple pregnancies, especially monochorionic ones. The type of chorionicity and amniality also affects the optimal timing of delivery. In uncomplicated pregnancies, delivery for monochorionic twins is recommended at 36–37 weeks, for dichorionic twins at 37–38 weeks, and for triplets at 35–36 weeks. The high risk of umbilical cord torsion and compression in monochorionic monoamniotic twins suggests delivery at 32–34 weeks

in a third-level hospital. With regard to delivery, in case of cephalic presentation of the first of dichorionic or monochorionic diamniotic twins, vaginal delivery is preferred after assessing additional risk factors with expanding indications for cesarean section. This review provides an analysis of the timing and method of delivery in multiple pregnancies, according to the latest international and domestic studies and clinical recommendations.
