The article describes a clinical case of corticobasal degeneration (CBD), a rare neurodegenerative disease. Over the past two years, a 68-year-old patient has been developing and gradually progressing parkinsonism syndrome with postural disorders, pyramidal insufficiency, oculomotor disorders, “alien” hand syndrome, and cognitive impairment of a predominantly frontal-subcortical nature with the presence of mixed apraxia. There is no response to levodopa drugs. Differential diagnosis with Parkinson's disease and atypical parkinsonism diseases was carried out. In accordance with the latest diagnostic criteria, a diagnosis was made: corticobasal degeneration, the clinical phenotype is progressive supranuclear palsy syndrome. Therapeutic approaches to patient management are covered

with CBD. The possibility of using the NMDA receptor inhibitor Akatinol Memantine in this pathology is discussed.
