Annotation:

Introduction. In Russia, the incidence of cervical cancer (CC) is 15.8 cases per 100 thousand women, which is much higher than in developed countries. Screening for cervical cancer during preventive examinations is not effective enough.

Objective: to determine the main directions for improving cervical cancer screening and evaluate the clinical and economic effectiveness of modern methods and programs.

Material and methods. The review included 82 publications found using keywords in the PubMed, MEDLINE, Google Scholar, Cochrane Library, and eLibrary databases. Search depth – 10 years (2012–2022).

Results. The HPV test is more effective in detecting CIN2/3+ compared to the PAP test (sensitivity 81.5 and 40.7%, respectively). Self-collection of material for HPV testing allows you to increase screening coverage of the target group by 2–4 times. Validation of the Qvintip device showed a sensitivity for detecting CIN2+ of 91%, for CIN3+ - 90%, specificity for CIN2/3+ - 98%. The accuracy of Qvintip self-collection compared to physician collection was similar at 1.03, with higher acceptability rates. Testing for high-risk HPV every 5 years with sequential genotyping and PAP testing is the most effective and least costly strategy.

Conclusion. The primary HPV test gives a better chance of detecting CIN2/3+ compared to the cytological method. The inclusion of the Qvintip device in a combined step-by-step program of organized screening in the regions of Russia will help improve the clinical and economic effectiveness of cervical cancer prevention.
