Annotation:

The purpose of the review is to analyze the pathogenetic aspects and therapeutic strategies of fatigue syndrome in patients with cerebrovascular pathology. Chronic fatigue is a common syndrome responsible for worsening the quality of life and prognosis of cerebrovascular disease. Mental fatigue is closely associated with cognitive impairment and may be considered

as an early clinical indicator of cerebral vascular disease. Fatigue syndrome and cognitive impairment are based on common mechanisms, including altered cortical excitability, inflammation, oxidative stress, and secondary mitochondrial failure. Oxidative stress and mitochondrial dysfunction may be considered important targets for therapeutic intervention. The antioxidant idebenone (Noben), a synthetic analogue of coenzyme Q10, is one of the promising pharmacological agents with pathogenetic potential.

The use of idebenone (Noben) for the treatment of the syndrome of cognitive impairment and fatigue in the complex therapy of cerebrovascular disease is justified and appropriate.
