EXPERIENCE OF ULTRASONIC CAVITATION IN THE TREATMENT OF LICHENOS SCLERONS IN GIRLS

Annotation:

One of the little-studied diseases in gynecology of children and adolescents is rightfully considered scleroatrophic lesions (N90.4 according to ICD-10) of the skin of the vulva and perianal area. Based on the results of a few studies, the multifactorial nature of this condition has been suggested, including the influence of autoimmune, infectious and hormonal influences. According to some authors, the period from the onset of complaints to the diagnosis of “lichen sclerosus of the vulva” in children can be from 1.5 to 2 years, during which specialists prescribe incorrect treatment, leading to a temporary positive effect. High prevalence of the disease, lack of rapid

the effect of the treatment, the chronicity of the process require the search for new methods of therapy.

Objective: to evaluate the effectiveness of ultrasonic cavitation in complex therapy of lichen sclerosus in girls.

Material and methods. The study included 60 girls aged 6–10 years with diagnosed lichen sclerosus of the vulva. Diagnosis was based on patient complaints and visual examination. Before the start of therapy and 10 days after the start of treatment, all girls underwent smears for flora and the Femoflor 16 test by scraping the posterolateral vaginal wall behind the hymenal ring. Next, the patients were divided into two groups: the first group consisted of patients who were treated with clobetasol propionate ointment 0.05% locally on the vulva and perianal area in a thin layer 2 times a day for 1 month; Patients of the second group, in parallel with ointment treatment, underwent a course of ultrasonic cavitation procedures (No. 5) using a solution of benzydamine hydrochloride 1 mg (140 ml) for 5 minutes.

Results. The results of the study showed that the addition of ultrasonic cavitation to the main therapy of lichen sclerosus in girls allows one to obtain a clinical effect within 5 days from the start of therapy. Also, in the main group of patients, a more rapid positive effect was noted in normalizing the vaginal biocenosis.
