PRECONCEPTIVE THERAPY FOR INFERTILITY ASSOCIATED WITH POLYCYSTIC OVARY SYNDROME

Annotation:

Anovulatory infertility, associated with the presence of polycystic ovary syndrome (PCOS), accounts for about 25% of all nosological forms of female factor. Patients with PCOS are characterized by a decrease in the quality of oocytes, which largely determines the limited effectiveness of infertility treatment, not exceeding 40%. Myo-inositol (MI) supplementation at the preconception stage is a promising strategy aimed at improving oocyte quality, however, resistance to this type of therapy exists, and solutions to this problem are needed.

Objectives: 1) evaluate the role of MI in improving the quality of oocytes in PCOS and the effectiveness of preconception therapy for MI; 2) study the state of the problem of refractoriness to MI therapy; 3) analyze possible ways to overcome resistance to MI therapy.

Material and methods. The review is based on the results of recent Russian and foreign studies published in the international databases MEDLINE and Embase, devoted to the effect of MI on the quality of gametes and metabolism in PCOS.

Conclusion. The subsidy of MI for PCOS at the preconceptional stage is quite effective in terms of correcting metabolic disorders, improving the quality of oocytes and increasing the chances of normal fertilization. Considering the presence of resistance to MI therapy in more than every third patient with PCOS, the need to find ways to solve this problem is beyond doubt. The combination of MI with alpha-lactalbumin showed very encouraging results in patients with PCOS and anovulation.
